The toolbar Plan chip is removed entirely: two adjacent Plan controls with different semantics was a UX trap, so there is now one control and one source of truth. /plan off exits the mode, mid-turn switches report as queued, failures surface the kernel reason, and the home composer explains the session-scoped usage. The PermissionModeSelector plan option (the approval gate) is deliberately untouched. tsc clean.
